Proof of Vaccination

If you are uploading your card within 24 hours before attending something at Beth Sholom, please bring your vaccination card with you on the day of the program or service. We will not receive it digitally in time.

NOTE: If you do not upload a vaccination card here or if the card does not show your name and your vaccinations, filling out this form accomplishes nothing. Please make sure to attach the correct documentation

Current Policies

The health and safety of our community is our highest concern.  We continuously monitor the COVID situation in our area and adapt our policies based on changing realities.  This page contains our current COVID-19 policies.

  • All attendees ages 5 and up must be vaccinated to attend ANY Beth Sholom program. "Program" includes all activities, classes, services, meetings, and other Beth Sholom sanctioned gatherings.

  • Proof of vaccination may be submitted here.

  • All attendees must remain masked indoors; masks may be removed at outdoor programs. Individuals at the bima alone (chanting a haftarah, offering a d’var Torah, a lecture, etc.) may remove their mask.

  • When there is food or drink involved at a program, attendees must remain masked while they are not eating or drinking. We will provide ample space and extra tables for all to spread out. Seating in the sukkah will be available as the weather permits.

  • Services will continue to be streamed through our website, and weekday minyanim will continue to be on Zoom.
